Parent / Guardian Consent

I, the undersigned parent or legal guardian, hereby give permission for my child listed above to attend and participate in Camp Ignite and all activities associated with the camp.

I understand that camp activities may involve physical activity and inherent risks. I voluntarily assume all such risks and agree to release, hold harmless, and indemnify Camp Ignite, its staff, volunteers, and affiliates from any liability for injury, illness, or loss that may occur during camp participation, except in cases of gross negligence.

Medical Authorization

In the event of an accident, injury, or illness, I authorize Camp Ignite staff to secure medical treatment for my child as deemed necessary. This includes, but is not limited to, first aid, emergency medical care, hospitalization, or treatment by a licensed physician.

I understand that reasonable efforts will be made to contact me prior to any medical treatment. However, if I cannot be reached, I authorize medical personnel to proceed with treatment in my child’s best interest.

Photo & Media Release

I grant permission to Camp Ignite to use photographs, video recordings, or other likenesses of my child for camp-related promotional materials, publications, social media, or website content, without compensation.
