I hereby grant permission for my child to attend Vacation Bible School (VBS). I agree to release and hold harmless Mountain Valley Baptist Church and/or its leadership and/or its employees and/or VBS volunteers from any liability for any accident or injury that may arise from my child’s participation in VBS.
